Conference Fees
Registration Fee before May 30 $300 per person
Registration Fee after May 30 $350 per person
Attending Spouse Fee $100
Please note the following:
  • The registration fee includes reception, meals, and conference materials.
  • Spouse fee includes the reception and conference meals.
  • A check payable to CHEA for the registration fee must accompany this form. No registration will be processed without payment.
  • In the event of registration cancellation, fees paid in advance are refundable — less a $50.00 service charge provided written notice is received by CHEA postmarked on or before May 30, 2000. There will be no refunds for cancellations received after May 30, 2000. Refunds will be returned after meeting. Refunds are not available in case of failure to attend.
